SUMMARY:Person-Centered Practices Knowledge\, Skills\, and Abilities for Community Care Hubs
DESCRIPTION:Person-centered practices help people live fully in their communities and work toward their own life goals. Supports should be designed around what each person wants and needs. Community care hubs (CCHs) are well-positioned to deliver this kind of support. They bring together community-based service providers and health care organizations to address factors that affect health and well-being. Frontline staff play a central role. They assess each person’s needs\, connect them to services and resources\, and create person-centered plans that reflect individual goals and desired outcomes. \nHosted by ACL’s No Wrong Door Resource Center\, this webinar will highlight two new resources that define the essential knowledge\, skills\, and abilities frontline community-based service providers need to effectively deliver person-centered support. The session will highlight Southern Alabama Regional Council on Aging’s Community Care Solutions and the promising practices this CCH uses to train the community workforce. \nThis webinar will be recorded\, and the recording link and materials will be made available to registrants afterward. ASL interpretation and English captions will be provided during the webinar. If you require any additional accommodations\, please email srajcevic@hsri.org. \nPanelists \n\nDave Crocker\, Community Care Solutions\, Southern Alabama Regional Council on Aging\nSaska Rajcevic\, Human Services Research Institute\nBevin Croft\, Human Services Research Institute\n\nRegister

SUMMARY:Addressing Food Insecurity for People Living Alone with Dementia
DESCRIPTION:National data indicate that nearly one quarter of older Americans living alone have dementia. This population faces numerous challenges\, including food insecurity\, which is often worsened by cognitive impairment\, limited social support\, and physical isolation. Traditional senior meal delivery programs are not always designed to address the unique needs of individuals with cognitive impairment\, who may have difficulty safely storing\, preparing\, or cooking meals. \nThis webinar will provide insight into an innovative\, volunteer-based food delivery model created to better support seniors living alone with dementia. Participants will learn about common barriers to food-based services experienced by individuals living alone with dementia\, the key components of an effective food delivery service model for people living alone with dementia\, and program evaluation best practices for measuring the impact of services for this population. \nAdditional information about this webinar and the registration form can be found here. \nPlease note: \n\nNational Alzheimer’s and Dementia Resource Center (NADRC) webinars are free of charge and open to the public.\nNADRC webinars are recorded and will be available for viewing at https://nadrc.acl.gov.\nPre-registration is required to attend the live webinars.\nClosed captioning is available during the live event and is included in the recording for all NADRC webinars.

SUMMARY:The 45% Hope: Brain-Health Levers That Help Keep the Lights On (Deaf-Friendly Version)
DESCRIPTION:In The 45% Hope\, Dr. Jaime A.B. Wilson reframes brain health through a powerful metaphor: your brain is not a switch… It’s a city with lights\, highways\, and dimmers. Research now suggests that up to 45% of dementia risk is modifiable\, meaning there are real\, practical ways to keep the lights on longer\, at any age. In this engaging\, Deaf-friendly webinar\, Dr. Wilson- a Prescribing Medical Psychologist and Board-Certified Neuropsychologist- breaks down the six evidence-based brain-health levers that matter most. With clear visuals\, real-world examples\, and an empowering message of hope over fear\, this session shows why small\, consistent changes… not extreme solutions… create the biggest impact for brain-health. Whether you’re an older adult\, caregiver\, professional\, or advocate\, this webinar will help you see your brain\, and your future\, with new clarity. \nRegister
